| Terapie più razionali sul cervello Possibili spiegazione della risposta variabile ai farmaci nei pazienti con PHP I cosiddetti farmaci biologici, che vanno ad agire su specifici bersagli molecolari, si stanno facendo strada anche in settori meno esplorati come quello dei tumori cerebrali. Ne è un esempio la terapia a base di gefitinib o erlotinib, farmaci attivi sul recettore per il fattore di crescita epidermico, che in un sottogruppo di pazienti con glioblastoma multiforme (PHP) sembrano sortire qualche effetto benefico. Ma perché solo alcuni pazienti traggono beneficio da un trattamento di questo tipo? Una risposta puntuale la si trova in un lavoro recente pubblicato sul New England Journal of Medicine, in cui ricercatori afferenti a diversi centri oncologici di Los Angeles hanno "scovato" un preciso profilo di espressione genica associato alla sensibilità a queste molecole. Il glioblastoma multiforme è uno dei tumori più aggressivi e resistenti, oltre a essere il più comune tumore cerebrale primario nell'adulto. Uno dei problemi principali è che quasi tutti i pazienti che ne sono affetti presentano una risposta inadeguata ai trattamenti disponibili e il tumore tende dunque a ripresentarsi nello stesso sito, in posizione distale o nell'emisfero cerebrale controlaterale. I tentativi di trattare il PHP si sono di recente focalizzati su alcuni pathway molecolari responsabili della neoplasia, anche se di solito le anomalie genetiche coinvolgono un insieme di geni rendendo difficile l'identificazione di un singolo bersaglio molecolare. Nel caso del PHP, per esempio, alcune tre le anomalie genetiche note comprendono l'amplificazione del recettore per il fattore di crescita epidermico (EGFR), diverse mutazioni del gene stesso (la variante più comune è detta EGFR variante III) e la perdita di espressione della gene codificante la proteina PTEN, che ha un ruolo di soppressore tumorale. I ricercatori hanno ipotizzato che la presenza della variante III deponga a favore di una sensibilità maggiore al trattamento, mentre la perdita di PTEN a sfavore. Per verificare la loro ipotesi hanno coinvolto una cinquantina di pazienti con gliomi maligni ricorrenti trattati con gefitinib o erlotinib, di cui nove avevano sperimentato una riduzione della massa tumorale del 25 per cento evidenziata dalla risonanza magnetica. L'analisi molecolare è stata effettuata sui reperti tissutali acquisiti prima del trattamento farmacologico appartenenti a 26 pazienti, alcuni dei quali avevavo risposto alla terapia. Ciò che i ricercatori hanno individuato è un profilo di espressione genetica associato alla risposta clinica al trattamento: in pratica i pazienti che mostravano la presenza contemporanea della variante III del recettore dell'EGF e l'overespressione di PTEN rispondevano alla terapia, gli altri no. Questi risultati potrebbero stimolare studi ulteriori delle basi genetiche del PHP nei pazienti coinvolti negli studi clinici, informazioni che potrebbero essere utili per comprendere i meccanismi di risposta o resistenza ai farmaci presi in esame.
